## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: WHILE OBSERVING ENGINE BEING REMOVED FROM AIRCRAFT, IT WAS DISCOVERED THAT THE P/C (PNEUMATIC) STAINLESS STEEL LINE HAD COME LOOSE FROM ENGINE. EVIDENCE SHOWED BLACK STREAKS ORIGINATING AT B NUT SLEEVE AND COVERING LINE ADJACENT TO "B" NUT. THERE WAS WEAR MARKS ON THE OUTBOARD THREADS OF FITTING INDICATING THAT THE LINE HAD NOT BEEN FULLY SEATED. WHEN IT FINALLY BACKED OFF, THE ENGINE SPOOLED DOWN AND FINALLY FAILED. (THE P/C LINE CONNECTS TO THE FUEL CONTROL AND THE ENGINE GEAR BOX) IT CAME LOOSE AT THE ENGINE END. THIS CASE IS FORWARDED TO FSDO 17, THE CHDO FOR THE OPERATOR FOR FURTHER INVESTIGATION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB MIA95LA065](https://openairdocs.org/ntsb/MIA95LA065), Jan 28, 1995: Bell 206-B3 (N5008N), Miami, FL, substantial damage. Probable cause: INADEQUATE MAINTENANCE AND INSPECTION, AND A LOOSE FUEL PC LINE. THE WIRES WERE A FACTOR WHICH NECESSITATED EARLY COLLECTIVE PITCH.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
