## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: THE PILOT DOES NOT REMEMBER IF HE OR THE PASSENGER MOVED OR DISLODGED THE GEAR HANDLE. IN A DISCUSSION WITH THE PILOT, MEASURES THAT COULD BE TAKEN TO PREVENT SOMETHING LIKE THIS OCCURRING AGAIN WERE DISCUSSED. HE STATED THAT HE PLANS TO MODIFY HIS CHECKLIST TO BE DEFINITELY CERTAIN THE GEAR LEVER IS IN THE PROPER POSITION BEFORE AIRCRAFT MOVEMENT. DURING THE TAKEOFF ROLL ON RUNWAY 22, THE AIRCRAFT REACHED 62 KNOTS, APPROXIMATELY 3 KNOTS UNDER THE NORMAL ROTATION SPEED OF 65 KNOTS. AT 62 KNOTS THE NOSE GEAR COLLAPSED AND THE AIRCRAFT SLID TO A STOP ON THE RUNWAY. POST-INCIDENT INVESTIGATION INCLUDED INSPECTION OF THE NOSE GEAR, INCLUDING A GEAR SWING. NOTHING SIGNIFICANT WAS FOUND.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
