## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON MARCH 13, 1995, AT APPROXIMATELY 1930 EST THE PILOT DEPARTED JACKSON AIRPORT AT JACKSON, MI AND AN UNUSUALLY LOUD NOISE WAS HEARD BELOW THE CABIN WHEN THE GEAR WAS RETRACTED. ALL INDICATIONS IN THE LANDING GEAR SYSTEM WERE NORMAL SO THE FLIGHT CONTINUED TO GRAND RAPIDS, MI. WHEN THE LANDING GEAR WAS SELECTED DOWN PRIOR TO LANDING, THE LANDING GEAR FAILED TO EXTEND. AND AN UNSAFE GEAR DOWN WAS INDICATED. THE EMERGENCY SYSTEM WAS USED WITHOUT ANY RESULTS. AFTER ATTEMPTING LANDING GEAR EXTENSION FOR TWO HOURS A GEAR UP LANDING WAS MADE.. CAUSING MAJOR IMPACT DAMAGE TO THE FUSELAGE ABOVE THE FLAPS. AIRWORTHINESS: THE FAILURE OF THE HALF RING CLIP ASSEMBLY WHICH SECURES THE WORM GEAR IN THE LANDING GEAR ACTUATOR ASSEMBLY, CAUSED THE SYSTEM FAILURE. FAILURE OF THE HALF RING CLIP WAS DUE TO FATIGUE FAILURE.

## NTSB investigation

None matched to this report by aircraft and date. Search the NTSB's own records: https://data.ntsb.gov/carol-main-public/basic-search

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
