## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-16) ON AUGUST 22, 1995, AT 1120 CST, AN AIRTRACTOR, N3169U WAS SUBSTANTIALLY DAMAGED WHILE TAKING OFF FROM A PLOWED FIELD NEAR DALHART, TX. NO INJURIES WERE SUSTAINED BY THE LONE OCCUPANT, A COMMERCIAL PILOT. VISUAL METEOROLOGICAL CONDITIONS PREVAILED FOR THE AGRICULTURAL FLIGHT. THE AIRMAN MEDICAL QUALIFICATIONS WERE INVOLVED BECAUSE THE PILOT'S MEDICAL HAD EXPIRED (EVEN AS A 3RD CLASS) ON 08/01/95. FURTHER, THE PILOT PRESENTED A MEDICAL CERTIFICATE TO THE INVESTIGATING INSPECTOR THAT HAD THE ISSUE DATE ALTERED. THIS MATTER IS UNDER INVESTIGATION.

## NTSB investigation

None matched to this report by aircraft and date. Search the NTSB's own records: https://data.ntsb.gov/carol-main-public/basic-search

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
