## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: MR. WILBERT J. TOVREA, CERTIFICATE #482487931, WAS THE PILOT-IN-COMMAND GIVING FLIGHT INSTRUCTION TO SUSAN E. HATHERILL, A STUDENT PILOT. HE ALLOWED MS. HATHERILL TO EXECUTE RECOVERY FROM A TAIL ROTOR MALFUNCTION. MR. TOVREA INITIATED THE LOSS OF TAIL ROTOR THRUST CONDITION BY CENTERING THE ANTI-TORQUE PEDALS. THE AIRCRAFT YAWED RIGHT. THE STUDENT USED AN IMPROPER RECOVERY BY ADDING EXCESS COLLECTIVE PITCH DURING THE ENGINE OUT. THE AIRCRAFT TOUCHED DOWN ON THE FORWARD PART OF THE SKIDS, ROCKED BACKWARDS, THEN THE MAIN ROTOR CONTACTED THE TAIL BOOM AND TAIL ROTOR DRIVE SHAFT LEAVING DENTS IN BOTH. THE INSTRUCTOR WAS UNABLE TO TAKE THE CONTROLS IN TIME TO AVOID THE INCIDENT. THIS INCIDENT IS CLOSED WITH THIS REPORT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
