## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON OCTOBER 16, 1995, AT 10:15 LOCAL TIME, A LOCKHEED L1011, N194AT REGISTERED TO SHAWMUT BANK CONNECTICUT, AND OPERATED BY AMERICAN TRANS AIR IND., A PART 121 OPERATOR, DEPARTED INDIANAPOLIS, INDIANA ENROUTE TO LAS VEGAS, NEVADA. ON TAKEOFF NUMBER 3 ENGINE FIRE WARNING LIGHT ILLUMINATED IN COCKPIT. NUMBER 3 ENGINE WAS SHUT DOWN, AIRCRAFT RETURNED TO INDIANAPOLIS (IND) WITHOUT FURTHER INCIDENT. VISUAL METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME AND IFR FLIGHT PLAN WAS FILED. NO INJURIES OR DAMAGE TO THE AIRCRAFT WAS SUSTAINED. EXAMINATION ON THE GROUND REVEALED THAT THE 6 O'CLOCK BORE SCOPE BLANKING PLATE WAS LOOSE, ALLOWING APPROXIMATELY 800 DEGREE FAHRENHEIT BLEED AIR TO BLOW ACROSS THE FIRE SENSING LOOP. AMERICAN TRANS AIR MECHANICS REPLACED THE BLANKING PLUG, HELICOIL, BOLTS AND PLATE. THIS INCIDENT IS CONSIDERED CLOSED.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
