## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: DURING APPROACH TO LAND ON RUNWAY 19L AT RICHARD L. JONES, JR., AIRPORT (RVS), TULSA, OKLAHOMA, THE PILOT LOWERED THE LANDING GEAR AND VERIFIED THREE GREEN "GEAR DOWN" LIGHTS AND CHECKED THE VISUAL INDICATOR WHICH ALSO SHOWED THE GEAR WAS DOWN. THE PILOT STATED THAT HE MADE A NORMAL LANDING AND DURING ROLL OUT THE NOSE GEAR COLLAPSED. AN AIR TRAFFIC CONTROLLER ON DUTY AT RVS TOWER REPORTED OBSERVING N5481D ON FINAL APPROACH AND THAT THE LANDING GEAR APPEARED TO BE DOWN AND IN PLACE. A MECHANIC WHO INSPECTED THE AIRCRAFT AFTER THE INCIDENT FOUND DAMAGE TO THE NOSE GEAR ACTUATING ROD THAT MAY HAVE OCCURRED FROM A PREVIOUS HARD LANDING. THIS INCIDENT IS CLOSED. ^PRIVACY DATA OMITTED^

Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
