## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: THE SKYDIVER EXITED THE AIRCRAFT AT 4,500 FEET OVER THE BEACH. THE MAIN PARACHUTE FAILED TO OPEN. THE RESERVE PARACHUTE FAILED TO OPEN. THE SKYDIVER LANDED IN THE WATER APPROXIMATELY 200 YDS OFF SHORE. THE BODY WAS FOUND THE FOLLOWING DAY BY FISHERMAN. THE BODY WAS RECOVERED BY THE FLORIDA MARINE PATROL. INVESTIGATION REVEALED THAT THE LIFE JACKET THE INDIVIDUAL WAS WEARING WAS WORN IMPROPER. THE WAIST BAND OF THE LIFE JACKET WAS OVER OR (OUTSIDE) THE PILOT CHUTE LINES THUS PREVENTING THE MAIN CHUTE AND THE RESERVE CHUTE TO OPERATE PROPERLY. INVESTIGATION ALSO REVEALED THAT THE MAIN CHUTE WAS REPACKED TWO (2) DAYS PRIOR, AND THE RESERVE CHUTE ON 9/27/95 BY QUALIFIED INDIVIDUALS.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
