## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON DECEMBER 12, 1995, VALUJET FLIGHT 224 DEPARTED DALLAS/FORT WORTH AIRPORT AT 0900 CST ENROUTE TO ATLANTA, GEORGIA. DURING CLIMB AT 5000 FEET ALTITUDE THE CREW HEARD A LOUD BANG AND NOTED VIBRATION COMING FROM THE NUMBER 2 ENGINE. THEY PERFORMED THE EMERGENCY SHUTDOWN PRODECURES AND DECLARED AN EMERGENCY WITH ATC, THEN RETURNED TO DALLAS/FORT WORTH AIRPORT LANDING AT 0925 CST WITH NO FURTHER INCIDENT. AFTER RETURNING TO THE GATE MAINTENANCE INSPECTED THE ENGINE AND FOUND THAT A CONTAINED TURBINE FAILURE HAD OCCURRED. THE ENGINE BORESCOPE INSPECTION REVEALED THAT A T1 BLADE SEPARATION OCCURRED, CAUSING FURTHER DAMAGE TO THE REMAINING TURBINE SECTIONS. THIS ENGINE HAD ACCUMULATED 2,795 HOURS AND WAS SCHEDULED TO BE REPLACED WITHIN A MATTER OF DAYS FOR ROUTINE TIME CHANGE. THE ENGINE WAS REPLACED AND THE AIRCRAFT RETURNED TO SERVICE. THE CONCERN WITH THIS ENGINE FAILURE STEMS FROM THE FACT THAT ANOTHER AIRCRAFT ALSO PURCHASED FROM TURKISH AIRLINES AND OPERATED BY VALUJET HAD AN ENGINE FAILURE THAT CAUSED THE LOSS OF THE AIRCRAFT. THE CHDO FOR VALUJET HAS REQUESTED THE TEAR DOWN REPORT AND ALL RECORDS FOR THIS ENGINE. THIS INCIDENT IS CLOSED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB FTW96IA066](https://openairdocs.org/ntsb/FTW96IA066), Dec 12, 1995: McDonnell Douglas DC-9-32 (N930VV), Dfw Airport, TX, incident, no damage. Probable cause: fatigue failure of a first stage, high pressure, turbine blade.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
