## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON DECEMBER 19, 1995, CONNIE KALLITTA (CKSA-AMERICAN INTERNATIONAL AIRWAYS) B-747-132, N625PL FLIGHT 761 DEPARTED LOS ANGELES, CA TO HONOLULU, HI. DURING LANDING AT HONOLULU INTERNATIONAL AIRPORT AT ABOUT 80 KNOTS WITH THE THRUST REVERSER (T/R) DEPLOYED, NO. 4 ENGINE SEIZED (N1, N2 AND OIL PRESSURE DROPPED TO ZERO). WHILE TAXIING TO THE GATE, THERE WAS A FIRE WARNING IN THE COCKPIT AND THE FLIGHT CREW PERFORMED ENGINE FIRE CHECK. OUTSIDE OBSERVERS SAW FLAME SHOOT OUT BETWEEN THE ENGINE COWLING AND THE PYLON ON THE OUTBOARD SIDE. ENGINE FIRE BOTTLES DID NOT DISCHARGE DUE TO DEFECTIVE FIRE BOTTLES' LEFT AND RIGHT SQUIBS. THE FIRE WAS EXTINGUISHED USING PORTABLE FIRE EXTINGUISHERS. BUSTER SARE, CKSA MAINT. SUPERVISOR WITH BOEING AND PRATT AND WHITNEY REPRESENTATIVES INSPECTED THE ENGINE AND COULD NOT DETERMINE THE CAUSE OF THE FIRE. THERE WAS NO INDICATION OF FUEL LEAKS, OIL LEAKS OR WIRES SHORTING OUT. THEY SUSPECTED THAT HYDRAULIC FLUID LEAKED OUT WHEN THE T/R WAS DEPLOYED CAUSING THE FIRE. THE AIRCRAFT WAS FERRIED TO LOS ANGELES FOR ENGINE CHANGE AND FURTHER INVESTIGATION. INCIDENT IWP1396005 NO PILOT DEVIATION NO NMAC CLOSED.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
