## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: THE PILOT WAS CLEARED TO TAXI & HOLD IN POSITION ON RUNWAY 25. THE PILOT READ BACK THE TAXI CLEARANCE. WHEN N9801V TAXIED TO THE ACTIVE RUNWAY THE PILOT IS ALLEGED TO HAVE DEPARTED AND TAKEOFF ON RUNWAY 25. UPON INVESTIGATION THE FOLLOWING FACTS WERE DETERMINED: 1) ATCT DID ISSUE A TAKEOFF CLEARANCE TO THE PILOT OF N9801V. 2) THE ATCT CONTROLLER DID NOT PHYSICALLY OBSERVE THE AIRCRAFT TAKEOFF RUNWAY 25. 3) THE CONTROLLER ONLY BECAME AWARE OF N9801V WHEN THE AIRCRAFT WAS ABEAM THE CONTROL TOWER. 4) THE CONTROLLER DID NOT BELIEVE THAT THE AIRCRAFT COULD BE AIRBORNE IN SUCH A SHORT DISTANCE AND TIME. 5) THE AIRCRAFT DEPARTED FROM 33 TAXIWAY WHICH IS 1800+ FEET TO THE BEAM POSITION OF THE ATC TOWER. 6) THE PERFORMANCE CHARTS FOR THE CE 172-M VERIFY THAT THE AIRCRAFT WOULD BE AIRBORNE IN 600 FEET. WITH THE TEMPERATURE AND WIND AT THAT TIME.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
