## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON APRIL 6, 1996, AT APPROX. 4 P.M., CESSNA N4576Q EXPERIENCED A RIGHT LANDING GEAR RETRACTION AFTER LANDING AT THE HILO AIRPORT. THE AIRCRAFT SUSTAINED MINOR DAMAGE TO THE PROP, LANDING GEAR DOOR AND LOWER GEAR STRUT, WING TIP AND AILERON. THE INCIDENT WAS CAUSED BY THE FAILURE THE CAST ALUMINUM FITTING ON THE END OF THE PUSH/PULL RETRACT TUBE. THIS FITTING HELD A BEARING IN PLACE THRU WHICH THE LANDING GEAR WAS EITHER PUSH FOR RETRACTION OR PULLED FOR EXTENSION. EXAMINATION OF THE FRACTURE SURFACE INDICATES A PREEXISTING CRACK. THIS PART, IF ORIGINAL, HAD OVER 18,000 HOURS TOTAL TIME AND MANY YEARS IN THE HAWAII CORROSIVE ENVIRONMENT. EXAMINATION ALSO REVEALED THAT THE BEARING HAD BEEN RESTAKED WITH A CENTER-PUNCH THAT PROBABLY CAUSED A STRESS RISER AT THE EDGE OF THE BEARING HOLE. THE FACTORY STAKES WERE DONE WITH A TOOL THAT WOULD NOT RESULT IN STRESS RISERS. NO LOG ENTRY COULD BE FOUND THAT WOULD TRACE THIS WORK TO A PERSON. THIS BEARING WAS NOT DESIGNED TO BE REPLACED IN THE FIELD. CESSNA HAD NO HISTORY ON FAILURES BUT DID STATE THEY SOLD A LOT OF THESE ASSEMBLIES. CLOSED. INCIDENT IWP 1396015 NO PILOT DEVIATION NO NMCA

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
