NARRATIVE: THE PILOT SAID HE SUSTAINED NO INJURIES. HE SUBMITTED THE FOLLOWING STATEMENT. NOTE; THE ORIGINAL SIGNED STATEMENT IS ON FILE AT NE-FSDO-03. "I DEPARTED SKYLARK AIRPORT AT APPROXIMATELY 4:10 P.M. LOCAL TIME. I PROCEEDED TO BAF AND WAS ASSIGNED RUNWAY 2 FOR LANDING WITH A RIGHT BASE. ON ABOUT 2 MILE RIGHT BASE I THOUGHT I PUT THE GEAR DOWN, BUT IN RETROSPECT, I REALIZE THAT I HAD PUT THE FLAPS DOWN BECAUSE WHEN I WENT TO PUT THE FLAPS DOWN, THEY WERE ALREADY DOWN. IT DID NOT DAWN ON ME THAT I HAD NOT PUT THE GEAR DOWN UNTIL I HAD MADE CONTACT WITH THE GROUND," SIGNED BY JAMES M. QUINN.
