## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THIS WAS A REGULARY SCHEDULED LUFTHANSA (DLH) FLIGHT FROM FRANKFURT TO DALLAS/FORT WORTH (DFW), CONTINUING TO HOUSTON (IAH). ALL OPERATIONS WERE NORMAL UNTIL THE TIME OF THE MISHAP. DURING THE DEPARTURE CLIMB FROM DFW, IN VISUAL METEOROLOGICAL CONDITIONS (VMC), AT APPROXIMATELY 13,000 FEET, HEADING 160, DLH 436 HAD A TCAS (TRAFFIC COLLISION AVOIDANCE SYSTEM) TRAFFIC ADVISORY (TA) FOR AN AIRCRAFT AT APPROXIMATELY "ONE O'CLOCK." ALMOST SIMULTANEOUSLY, A VFR (VISUAL FLIGHT RULES) TWIN CESSNA AIRCRAFT, HEADING NORTH AT 14,500 FEET, CALLED ATC REQUESTING TRAFFIC ADVISORIES. DLH 436 THEN HAD A TCAS RESOLUTION ALERT (RA) WHICH PROMPTED THE PILOTS OF DLH 436 TO INITIATED EVASIVE ACTION TO AVOID A POSSIBLE MIDAIR COLLISION. DURING THIS MANEUVER, SUBSTANTIAL POSITIVE AND NEGATIVE "G" FORCES WERE INDUCED IN THE TAIL SECTION OF THE A340 AIRCRAFT, WHICH TOSSED ABOUT AND SERIOUSLY INJURED SEVERAL FLIGHT ATTENDANTS AND ONE PASSENGER. THE LUFTHANSA PILOTS NEVER SAW THE TWIN CESSNA AIRCRAFT. BY THE TIME ATC HAD IDENTIFIED THE TWIN CESSNA ON RADAR, THE TWO AIRCRAFT HAD PASSED CLEAR OF EACH OTHER, AND THE TWIN CESSNA PILOT ACKNOWLEDGED HAVING HAD THE LUFTHANSA A340 IN SIGHT. AFTER DETERMINING THAT PEOPLE ON THE AIRCRAFT WERE INJURED, DLH 436 REQUESTED EXPEDITIOUS ATC CLEARANCE TO IAH, TO LAND ON RUNWAY 08, AND FOR MEDICAL ASSISTANCE TO MEET THE AIRCRAFT AT THE INTERNATIONAL ARRIVAL GATE.

## NTSB investigation

None matched to this report by aircraft and date. Search the NTSB's own records: https://data.ntsb.gov/carol-main-public/basic-search

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
