## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: DURING TAKE OFF CLIMB THE PILOT WAS NOTIFIED BY FARGO, ND TOWER THAT THE RIGHT OUTBOARD MAIN GEAR TIRE HAD FALLEN OFF. THE AIRCRAFT WAS DEPARTING FARGO, ND IN ROUTE TO BISMARCK, ND AND THEN WILLISTON, ND. PILOT CYCLED LANDING GEAR WITHOUT ANY PROBLEMS. GREAT LAKES AVIATION MAINTENANCE CONTROL TOLD PILOT TO FLY DIRECTLY TO WILLISTON, ND AND NOT TO STOP IN BISMARCK. THE AIRCRAFT LANDED IN WILLISTON, ND WITHOUT ANY FURTHER INCIDENT. AIRWORTHINESS WAS INVOLVED BECAUSE THE BEARINGS IN THE WHEEL FAILED AND THE WHEEL ASSEMBLY DEPARTED THE AIRCRAFT. THIS WAS THE THIRD INCIDENT OF THIS KIND IN THE PAST 3 WEEKS. THE PRINCIPLE MAINTENANCE INSPECTOR, KEN RIEGER OF THE DES MOINES FSDO IS IN THE PROCESS OF FOLLOWING UP ON THIS PROBLEM. GREAT LAKE AVIATION HAS ISSUED A FLEET CAMPAIGN DIRECTIVE TO INSPECT ALL AIRCRAFT IN THEIR FLEET. ALSO, THEY ARE LOOKING INTO THE MAINTENANCE PROCEDURE AND TYPE OF GREASE BEING USED.

Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
