## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON SATURDAY, AUGUST 17, 1996 AT APPROXIMATELY 1940 EST, MR. CHARLES A. MAIDMENT WAS FLYING HIS PARAPLANE MODEL PSE-2 WHEN WITNESSES SAW THE RIGHT PORTION OF THE PARACHUTE COLLAPSE. THE WITNESSES STATED THE PARACHUTE STARTED TO SPIN AND DROPPED DOWN INTO A BEAN FIELD. MR. MAIDMENT WAS TRANSPORTED TO A LOCAL HOSPITAL WHER HE DIED A FEW HOURS LATER DUE TO MASSIVE INTERNAL INJURIES. THE PARAPLANE INVOLVED IN THIS INCIDENT DID NOT, AND WAS NOT REQUIRED TO HAVE AN FAA REGISTRATION CERTIFICATE ON BOARD. AFTER A DATABASE SEARCH, IT WAS DETERMINED THAT MR. MAIDMENT DOES NOT HOLD A PILOT CERTIFICATE. SINCE THE PARAPLANE WAS OPERATED AS AN ULTRALIGHT AND WAS NOT REQUIRED TO HAVE A REGISTRATION CERTIFICATE, AND A FATAL INJURY WAS RECEIVED, THIS INCIDENT RECORD IS BEING MADE ONLY FOR TRACKING PURPOSES.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
