## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-16) THE OIL FILLER CAP WAS IMPROPERLY REINSTALLED AFTER THE ENGINE OIL WAS CHANGED. THE PILOT CHECKED THAT THE CAP WAS INSTALLED BUT FAILED TO DETERMINE THAT IT WAS PROPERLY SECURED. THE CAP IS SECURED TO THE FILLER NECK BY A CHAIN ATTACHED TO A METAL RING. IF THE CAP IS NOT PLACED ON THE FILLER NECK PROPERLY, THE CHAIN ONLY ALLOWS THE CAP TO TURN A SMALL AMOUNT-NOT ENOUGH TO ALLOW IT TO LOCK INTO POSITION WHICH IS IDENTIFIED BY A DETENT. THIS DESIGN ALLOWED THE CAP TO APPEAR TIGHT DURING THE PREFLIGHT CHECK BUT WHILE TAXIING THE CAP LOOSENED UNTIL FINALLY COMING COMPLETELY OFF AFTER TAKEOFF RESULTING IN THE LOSS OF THE ENGINE OIL. AN M OR D REPORT IS BEING PREPARED CONCERNING THIS STYLE OF CAP. THE SUBSTANTIAL DAMAGE TO THE AIRCRAFT AND THE DAMAGE TO THE TAXIWAY LIGHTS WAS A DIRECT RESULT OF THE DECISIONS AND PROCEDURES THE PILOT IMPLEMENTED OR FAILED TO IMPLEMENT DURING THE INCIDENT.^PRIVACY DATA OMITTED^
