## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: DURING TAKE-OFF FROM RUNWAY 5 AT THE LENAWEE COUNTY AIRPORT (ADG), THE PILOTS LEFT FOOT SLIPPED OFF THE RUDDER PEDAL, CAUSING THE AIRCRAFT TO ROLL TO THE RIGHT. THE MAIN ROTOR CONTACTED THE GROUND AND THE RIGHT DOOR, THE WINDSHIELD, THE CABIN ENCLOSURE, THE MAIN ROTOR FLIGHT CONTROLS AND THE PROPELLER WERE DAMAGED. RUDDER PEDAL DESIGN SHOULD BE CHANGED TO LESSEN THE POSSIBILITY OF LOSS OF RUDDER CONTROL DURING FLIGHT. MR. WATKINS ALLOWED HIS LEFT FOOT TO COME OFF THE RUDDER PEDAL, CAUSING LOSS OF DIRECTIONAL CONTROL.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
