## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON JUNE 16, 1997, FSDO-19 RECEIVED A CC-MAIL MESSAGE FROM AEA-200, CONCERNING AN INCIDENT WHICH OCCURRED ON JUNE 14, 1997. OUR INVESTIGATION REVEALED THAT A DC-9-31, TAIL NUMBER N958VJ, OPERATING AS US AIRWAYS FLIGHT #648 HAD THE #2 MAIN WHEEL FAIL AT PHILADELPHIA. THE FAILED WHEEL AND SUBSEQUENT FLAT TIRE WAS FOUND BY US AIRWAYS MAINTENANCE PERSONNEL DURING ROUTINE WALK AROUND. FURTHER INVESTIGATION REVEALED N958VJ WAS REMOVED FROM EXTENDED STORAGE AND RETURNED TO REVENUE SERVICE IN AUGUST 1996. A REVIEW OF THE MAINTENANCE RECORDS REVEALED SINCE AUGUST 1996 THIS AIRCRAFT HAS HAD FOUR #2 MAIN TIRES CHANGED DUE TO WEAR. THE WHEEL THAT FAILED WAS INSTALLED ON APRIL 29, 1997, AND REMAINED ON THE AIRCRAFT FOR APPROXIMATELY 47 DAYS. THE #2 WHEEL THAT FAILED WAS NOT THE SAME WHEEL THAT WAS ON THE AIRCRAFT WHEN IT WAS REMOVED FROM STORAGE. THE FAILED WHEEL WAS RED-TAGGED AND SHIPPED TO THE US AIRWAYS WHEEL AND TIRE FACILITY IN GREENSBORO, NORTH CAROLINA. THE WHEEL WILL UNDER GO A METALLURGICAL ANALYSIS TO ASCERTAIN THE PROBABLE CAUSE OF THE FAILURE. FAA INSPECTORS FROM FSDO-19 WILL INSPECT THE WHEEL IN GREENSBORO ON JULY 1, 1997.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
