## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

NARRATIVE: ON 9-24-97 A FRONTIER AIRLINES FLIGHT 776 SLC TO DEN DURING TAKE OFF LOST ALL "A" HYDRAULIC FLUID. THE PILOT DECLARED AN EMERGENCY AND LANDED ON RWY 16L DURING THE LANDING ROLL THE AIRCRAFT VEERED OFF THE RUNWAY TO THE RIGHT BETWEEN TAXIWAY H3 AND H4. AN EMERGENCY EVACUATION OF THE AIRCRAFT WAS INITIATED BY THE CAPTAIN. THERE WERE NO INJURIES DURING THE EVACUATION. THE LEFT AND RIGHT FORWARD ENTRY SLIDES, THE RIGHT AFT SLIDE, THE LEFT AND RIGHT OVER WING EXITS WERE ALL USED. THE ONLY EXIT NOT USED WAS THE LEFT AFT ENTRY DOOR WHICH HAS AN AIRSTAIR DOOR WHICH FLIGHT ATTENDANT ELECTED NOT TO USE. THERE WAS NO PARTICULAR EXIT USED BY THE MAJORITY OF THE EVACUEES. ACCORDING TO THE SALT LAKE CITY AIRPORT FIRE CAPTAIN EISENHOWER THE EVACUATION WAS COMPLETED IN LESS THAN TWO MINUTES. WHEN THE PILOT USED THE THRUST REVERSER ONLY THE RIGHT REVERSER DEPLOYED AND DID NOT STOW COMPLETELY WHEN THE PILOT STOWED THE THRUST REVERSER LEVER. BECAUSE THE RIGHT THRUST REVERSER DID NOT STOW THIS ADDED DRAG TO THAT SIDE AND THE PILOT WAS UNABLE TO STOP THE RIGHT DRIFT AS THE AIRCRAFT SLOWED AND THE RUDDER BECAME INEFFECTIVE. THE LEFT ENGINE LOW OIL PRESSURE SWITCH FAILED WHICH PREVENTED THE THRUST REVERSER IN THE LEFT ENGINE TO DEPLOY. THIS PART WAS REPLACED AND SENT TO THE MANUFACTURER TO DETERMINE THE CAUSE OF THE MALFUNCTION. AN M OR D REPORT WILL BE SUBMITTED ON THIS SWITCH BY FRONTIER AIRLINES. ^PRIVACY DATA OMITTED^ THIS IS THE FINAL REPORT.
