## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

MISS SANDY WAMBACH, A PARACHUTIST, WAS FATALLY INJURED WHEN SHE STRUCK THE GROUND. THE MAIN AND RESERVE PARACHUTES HAD NOT BEEN DEPLOYED. I WITNESSED THE EXAMINATION OF HER PARACHUTE BY THE OWNER AND OPERATOR OF SKY DIVE CHICAGO, ROGER NELSON. I VERIFIED THAT BOTH SYSTEMS IN HER PARACHUTE WERE FULLY FUNCTIONAL. A WITNESS REPORTED THAT MS. WAMBACH COLLIDED WITH ANOTHER JUMPER AND SHE WS RENDERED UNCONSCIOUS AND UNABLE OT WORK THE PARACHUTE CONTROLS. (ABOVE NARRATIVE FROM 8020-23)

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
