## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) COMPLETED INCIDENT INVESTIGATION OF ENGINE FAILURE DURING TAKEOFF ROLL OF AMERICAN AIRLINES FLIGHT 612, JULY 31, 1998. PREVIOUSLY INVESTIGATED AS AN "OCCURRENCE" RECEIVED FROM REGIONAL OPERATIONS CENTER. "AMERICAN AIRLINES FLIGHT 612, B727, ABORTED TAKEOFF AT O'HARE AIRPORT ON RUNWAY 09L AT 311655Z, DUE TO NUMBER THREE ENGINE FAILURE (NO REPORTS OF AN UNCONTAINED FAILURE). THE RUNWAY WAS CLOSED AND THE AIRCRAFT TAXIED TO THE GATE. INSPECTED THE AIRCRAFT IN THE HANGAR, AND REVIEWED LOGBOOK ENTRY "NBR 3 ENG FAILED ON T/O BELOW 70 KTS. LOUD BANG FELT IN THROTTLES. RETURNED TO GATE." ENGINE DAMAGE WAS BEING EVALUATED AT TIME OF OCCURRENCE INVESTIGATION AND THE ENGINE WAS SUBSEQUENTLY CHANGED. REVIEW OF LOGBOOK REVEALED NO RELATED DISCREPANCIES, ENGINE FAILURE WAS "CONTAINED". NO FURTHER ACTION REQUIRED.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
