## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) SKYWEST 90, A BRASILLIA E120, SANTA MARIA, CA, TO LAX, DIVERTED TO SANTA BARBARA, CA, BECAUSE OF ENGINE FAILURE. ARRIVED SANTA BARBARA MUNICIPAL AIRPORT AT 230925 PST. SMX-LAX, DIVERTED TO SBA DUE TO ENGINE FAILURE. CALLED MR. PHIL SHANNON, SKYWEST ST. GEOARGE MX, RE: THIS INCIDENT. ACCORDING TO MR. SHANNON, LAX MECHANICS WERE FLOWN TO SBA WHERE A/C WAS INSPECTED AND FOUND TO HAVE "LOOSE CONNECTIONS IN THE LEADING EDGE" BLEED AIR DUCT. THOSE CONNECTIONS WERE TIGHTENED, A/C WAS OPS CHECKED AND FOUND TO BE SATISFACTORY. CONVERSATION WITH MR. SHANNON CONTINUED WHERE HE SAID ON 4 MAR 99 THE SAME A/C, N578SW, EXPERIENCED A "DUCT LEAK" LIGHT ON, BUT NO FURTHER DETAILS WERE GIVEN OTHER THAN TO SAY THAT LAX MX WAS INVOLVED AND DISCREPANCY WAS SIGNED OFF AS CND (COULD NOT DUPLICATE) AFTER BLEED AIR SYSTEM WAS INSPECTED. THE THIRD INCIDENT CONCERNING THIS A/C, N578SW, OCCURRED ON 9 MAR 99 WHEN ANOTHER "DUCT LEAK" LIGHT CAME ON SHORTLY AFTER TAKEOFF FROM SBA, THE LEFT ENGINE WAS SHUT DOWN, AND A/C RETURNED TO SBA. LAX MX WAS FLOWN TO SBA, WHERE DE-ICE PRESSURE REGULATOR WAS FOUND TO BE FAULTY, REG WAS R&R'D, AND A/C FERRIED TO PSP FOR A COMPLETE CHECK OF THE BLEED AIR SYSTEM IN CLUDING THE DUCTS, DUCT CLAMPS, TEMP SENSORS, AND PRESS REG'S. SYSTEM INTEGRITY FOUND TO BE SATISFACTORY AND A/C RETURNED TO SERVICE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
