ON MARCH 24, 1999, AT ABOUT 0950, UNITED AIRLINES FLIGHT 212, AN AIRBUS A-320, TOOK OFF FROM LAX TO IAD WITH A CREW OF 6 AND 90 PASSENGERS. DURING THE TAKE-OFF ROLL, NUMBER 1 ENGINE FAN COWL OPENED AND AT LIFT OFF, THE INBOARD AND OUTBOARD FAN COWLS WERE TORN OFF THE ENGINE. THE ENGINE REMAINED AT TAKE-OFF POWER UNTIL A SUITABLE ALTITUDE WAS REACHED AND THEN IT WAS SHUT DOWN BY THE FLIGHT CREW. THE AIRCRAFT RETURNED TO LAX AND AN OVERWEIGHT LANDING WAS ACCOMPLISHED. INSPECTION OF THE AIRCRAFT REVEALED EXTENSIVE DAMAGE TO THE NUMBER 1 PYLON, NUMBER 1-3-4 AND 5 SLATS ON THE LEFT WING, WING LEADING EDGE UNDER THE SLATS, THE LEFT SIDE OF THE FUSELAGE FORWARD OF THE WING ROOT FAIRING AND THE HORIZONTAL STABILIZER INBOARD AND CENTER SECTION LEADING EDGES. PRELIMINARY INVESTIGATION INDICATES MAINTENANCE HAD BEEN PERFORMED ON THE NUMBER 1 ENGINE PRIOR TO THE DEPARTURE OF FLIGHT 212 PRIVACY DATA OMITTED ^PRIVACY DATA OMITTED^ AN INVESTIGATION IS UNDERWAY BY UALA AND THIS OFFICE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
