## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

MR. CHARLES RHOADES, PILOT IN COMMAND OF N80KP, MADE AN EMERGENCY LANDING AT THE WATERLOO AIRPORT. PILOT WAS UNABLE TO EXTEND THE NOSE GEAR. THIS WAS A MECHANICAL FAILURE. THE COTTER PIN WAS MISSING FROM THE CASTELLATED NUT OF THE NOSE GEAR DOWN LOCK RETAINING BOLT. THE CASTELLATED NUT HAD BACKED OFF THE BOLT TO THE END OF THE THREADS. THIS ALLOWED THE DOWN LOCK TABS TO PASS DURING THE OPERATION OF THE LANDING CYCLE. THIS INCIDENT IS CONSIDERED CLOSED WITH THIS REPORT. INCIDENT #ICE01990026

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
