As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE RIGHT LANDING GEAR COLLAPSED ON TAKE OFF FROM THE WILLOWS AIRPORT IN CA. AFTER THE AIRCRAFT LIFTED OFF THE GROUND AND GAIN ALITUDE THE PILOT CYCLED THE LANDING GEAR AND IT SEEMED TO WORK AS DESIGNED. THE PILOT FLEW TO HOME BASE AT THE PETALUMA AIRPORT, CA WITH THE LANDING GEAR IN THE DOWN POSITION. A SOFT LANDING WAS PERFORMED AND AFTER A SHORT ROLL THE RIGHT GEAR COLLAPASED, THEN THE LEFT MAIN AND NOSE GEAR COLLAPSED. THE AIRCRAFT SLID TO A STOP THERE WAS NO FIRE AND NO IMPACT WITH OBJECTS ON THE GROUND.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
