## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) JAMES BENEDICT FELL TO HIS DEATH DURING A SKYDIVING INCIDENT. HE JUMPED FROM THE AIRCRAFT AT APPROXIMATELY 3,500 FEET. AN INSPECTION OF THE PARACHUTING EQUIPMENT REVEALED THAT THE RESERVE WAS PARTIALLY DEPLOYED. NEITHER THE MAIN CANOPY ACTIVATION DEVICE NOR THE RESERVE CANOPY ACTIVATION DEVICE HAD BEEN ACTIVATED DURING DESCENT. THE INSPECTION ALSO INDICATED THATTHE HARNESS/CONTAINER SYSTEM MAY HAVE BEEN RETURNED TO SERVICE BY THE MASTER PARACHUTE RIGGER WITHOUT EVIDENCE OF A TSO, ^PRIVACY DATA OMITTED^ THE NATIONAL TRANSPORTATION SAFETY BOARD WAS NOTIFIED. THE NTSB INVESTIGATOR STATED THAT THEY DO NOT GET INVOLVED WITH PARACHUTING INCIDENTS.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
