## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23)THE PILOT LOST CONTROL ON LIFT OFF. THE AIRCRAFT MADE A HARD SWERVING RIGHT TURN AND LANDED ON RIGHT SIDE RESULTING IN SUBSTANTIAL DAMAGE. THE AIRCRAFT HAD TWO PASSENGERS ON BOARD, BOTH WEIGHTING OVER 200 POUNDS. THE PILOT WEIGHED APPROXIMATELY 300 POUNDS AND THE FUEL WAS NEARLY TO THE TOP. (.4) ACCORDING TO THE PILOT AND ONE PASSENGER, THE ROTORCRAFT LIFTED OFF THE AIRSTRIP TO ABOUT A 2 TO 3 FOOT HOVER, AND IMMEDIATELY COMMENCED A VIOLENT RIGHT SPIN THAT TERMINATED WITH THE LANDING SKID AND NOSE COLLIDING WITH THE TERRAIN. THE MAIN ROTOR SEVERED THE TAIL BOOM AND COLLIDED WITH THE TERRAIN, RESULTING IN THE ROTORCRAFT ROLLING OVER ON ITS RIGHT SIDE. THE FAA REPORTED THE WRECKAGE REVEALED NO EVIDENCE OF MECHANICAL FAILURE, AND THAT THE ROTORCRAFT WAS WITHIN ITS MAXIMUM TAKEOFF WEIGHT AND LOADING LIMITS. THE DENSITY ALTITUDE WAS 2,206 FEET.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB MIA00LA227](https://openairdocs.org/ntsb/MIA00LA227), Jul 22, 2000: Bell 206A-1 (N136C), Bennettsville, SC, substantial damage. Probable cause: The pilot's failure to use proper remedial action to counteract a loss of tail rotor effectiveness resulting in collision with terrain during the subsequent uncontrolled descent.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
