## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) INVESTIGATION REVEALED THAT THE 5/8" STEEL CABLE (LONG LINE) HAD FAILED BETWEEN THE HELICOPTER SLING HOOK AND THE EXTERNAL SLING LOAD (LOGS) AT THE TIME THE EXTERNAL LOAD WAS TO BE RELEASED. THE CABLE PORTION THAT WAS STILL CONNECTED TO THE HELICOPTER HOOK RECOILED FROM THE SEPARATED END AND ENTERED THE MRB ROTATION DISK AREA AND PIECES OF THE CABLE WERE SLUNG BACK INTO THE TRB DISK AREA. THE PILOTS EXECUTED A POWER-OFF LANDING. INSPECTION OF THE AIRCRAFT REVEALED ONE MRB WITH TWO DAMAGED POCKETS (TWO SEGMENTS OF BLADE AREA BETWEEN THE MRB SPAR AND MRB TRAILING EDGE) AND THE TAIL ROTOR ASSEMBLY WITH SUBSTANTIAL DAMAGE. NO OTHER DAMAGE NOTED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ANC01LA109](https://openairdocs.org/ntsb/ANC01LA109), Aug 11, 2001: Sikorsky S61A (N5193Y), Wrangell, AK, substantial damage. Probable cause: The separation of a steel long line cable that failed and rebounded from a load of logs into the helicopter's main and tail rotors.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
