THE FLIGHT ORIGINATED AT SAN ANTONIO, TEXAS ON SEPT. 25, 2001, AT APPROXIMATELY 11:20 AM. THE CAUSE OF THE EMERGENCY WAS DUE TO A BENT STEM IN THE DIAPHRAGM OF THE FUEL SERVO. DURING THE INTERNAL INSPECTION OF THE FUEL SERVO, IT WAS DISCOVERED THAT THE DIAPHRAGM CONTAINED SOME FOREIGN MATTER, (POSSIBLY DIRT). IT APPEARS THAT THE FOREIGN MATTER CONTRIBUTED TO THE BENDING OF THE STEM OF THE DIAPHRAGM.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
