## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE INSTRUCTOR AND STUDENT WERE PRACTICING EMERGENCY PROCEDURES AT THE COUNCIL BLUFFS, IA AIRPORT. THE STUDENT INITIATED A TAKE OFF FROM A HOVER. THE INSTRUCTOR ANNOUNCED "SIMULATED ENGINE FAILURE" AND ROLLED THE THROTTLE TO IDLE AS THE AIRCRAFT BEGAN TO MOVE FORWARD DURING THE TAKE OFF. THE STUDENT MAINTAINED PROPER DIRECTIONAL CONTROL, BUT IMMEDIATELY APPLIED EXCESSIVE AFT CYCLIC. THE INSTRUCTOR TOOK CORRECTIVE ACTION, BUT COULD NOT RECOVER FROM THE EXTREME TAIL LOW ATTITUDE CAUSED BY THE STUDENTS IMPROPER CONTROL INPUT. DUE TO CLOSE PROXIMITY TO THE GROUND, THE STINGER AND THE TAIL ROTOR CONTACTED THE PAVEMENT, CAUSING MINOR DAMAGE TO THE TAIL ROTOR BLADES. THIS INCIDENT IS CLOSED WITH THIS REPORT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
