(-23) ON NOVEMBER 22, 2001, AIRCRAFT N5UJ, A LEAR 25B, CRASHED ON TAKEOFF AT PITTSBURGH INTERNATIONAL AIRPORT. THE AIRCRAFT WAS ON A TITLE 14 PART 91 FLIGHT TO ITS BASE OF OPERATION. AN IN DEPTH INVESTIGATION HAS NOT REVEALED ANY MECHANICAL FAILURES AT THIS TIME. THE CREW SUSTAINED FATAL INJURIES AND THE AIRCRAFT WAS DESTROYED IN THE POST CRASH FIRE. SUBMISSION OF THIS REPORT CLOSES THIS INVESTIGATION.
