## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) LACK OF PROPER COMMUNICATIONS BETWEEN PILOT AND DECK HANDLING CREW. PILOT ATTEMPTED TO TAKE OFF WITH TIE DOWN STRAP AND APU CABLE STILL ATTACHED. AIRCRAFT (HELO) WENT OVER SIDE OF SHIP AND INTO WATER. AIRCRAFT DESTROYED AND SANK. MINOR INJURY TO PILOT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX02LA032](https://openairdocs.org/ntsb/LAX02LA032), Nov 23, 2001: McDonnell Douglas 369HS (N4265D), Pacific Ocean (Pacific Ocean), destroyed. Probable cause: A dynamic roll over due to the pilot's inadequate preflight inspection and failure to ensure that all tiedowns were removed. A factor in the accident may have been the lack of clear communications…

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
