## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(.19) ON JANUARY 12, 2002, AT 1430 CENTRAL STANDARD TIME, AN EXPERIMENTAL WOLTER AVID FLYER, N4047M, SUSTAINED SUBSTANTIAL DAMAGE WHEN IT DEPARTED CONTROLLED FLIGHT FROM 300 FEET ABOVE GROUND LEVEL (AGL) AND IMPACTED THE TERRAIN. THE PILOT RECEIVED FATAL INJURIES. THE 14 CFR PART 91 PERSONAL FLIGHT DEPARTED RUNWAY 34 (4,400 FEET BY 75 FEET, ASPHALT) AT RUSH CITY REGIONAL AIRPORT (ROS), RUSH CITY, MINNESOTA, ON A LOCAL PRACTICE FLIGHT. VISUAL METEOROLOGICAL CONDITIONS PREVAILED AND NO FLIGHT PLAN WAS FILED. THE EXPERIMENTAL AIRPLANE'S ORIGINAL FAA REGISTERED "N" NUMBER WAS N4047M. ## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI02LA062](https://openairdocs.org/ntsb/CHI02LA062), Jan 12, 2002: Wolter Avid Flyer (N4047M), Rush City, MN, substantial damage, 1 fatal. Probable cause: The unqualified pilot inadvertently stalled the airplane. A factor was the unqualified pilot's lack of total experience.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
