## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) DURING DEICING AT DFW TYPE I DEICING FLUID AND POSSIBLY ICE RUNOFF WAS INGESTED INTO THE APU INLET. THE SUBSTANCE INGESTED CAUSED THE APU TO OVERSPEED AND RESULT IN THE TURBINE WHEEL BREAKING APART AND LIBERATING FROM THE APU. FRAGMENTS PENETRATED THE AFT PRESSURE BULKHEAD IN THREE LOCATIONS WITH ONE LARGE PIECE COMING TO REST IN A FIRST AID KIT STORED BENEATH THE AFT FLIGHT ATTENDANTS JUMP SEAT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB FTW02LA088](https://openairdocs.org/ntsb/FTW02LA088), Mar 6, 2002: Fokker F-28 MK-100 (N1425A), Dallas, TX, substantial damage. Probable cause: The ground crew inadvertent application of deicing fluid in the auxiliary power unit resulting in an overspeed and turbine wheel burst. A contributing factor was the gusty wind.
