## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) N686Q DEPARTED JUNEAU ON RUNWAY 08, WITH A LOAD OF APPROXIMATELY 77 BUNDLES OF WOODEN ROOFING SHAKES, AT AN AVERAGE WEIGHT OF 56 POUNDS PER BUNDLE, BOUND FOR TAKU GLACIER LODGE. THIS WAS THE AIRCRAFTS SECOND TRIP OF THE DAY IN WHICH, ON THE FIRST TRIP, MISCELLANEOUS CARGO WAS DELIVERED. AS THE AIRCRAFT APPROACHED THE DEPARTURE END OF THE RUNWAY, AT APPROXIMATELY 200 TO 300 FEET ABOVE THE GROUND, THE NOSE OF THE AIRCRAFT PITCHED UP TO ABOUT 70 DEGREES AND ROLLED TO THE RIGHT. THE AIRCRAFT CONTINUED TO THE RIGHT AS THE NOSE OF THE AIRCRAFT LOWERED MOMENTARILY. AS THE AIRCRAFT FLEW VERY SLOWLY, IN A SOUTHERLY DIRECTION, THE LANDING GEAR WAS DEPLOYED. THE NOSE OF THE AIRCRAFT PITCHED UP AGAIN, ROLLED HARD TO THE RIGHT AND DESCENDED. DURING THE DESCENT THE WINGS ROCKED TO THE LEFT AND RIGHT, ONE WITNESS EXPLAINED, "LIKE A CARD IN THE WIND." THE AIRCRAFT CONTINUED ITS DESCENT INTO AN AREA OF TIDAL MUD FLATS AND SUBSEQUENTLY COLLIDED WITH SHALLOW OCEAN TIDAL WATERS. THE PILOT AND SOLE OCCUPANT OF THE AIRCRAFT RECEIVED FATAL INJURIES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ANC02FA028](https://openairdocs.org/ntsb/ANC02FA028), Apr 11, 2002: Beech E18S (N686Q), Juneau, AK, destroyed, 1 fatal. Probable cause: The pilot's excessive loading of the airplane that precipitated an inadvertent stall/mush during the initial climb.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
