## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON 04/10/2002, I WAS CALLED TO PARTICIPATE IN THE EQUIPMENT INVESTIGATION OF A FATAL PARACHUTE ACCIDENT. WITH THE HELP OF MR. T.K. DONLE, PROGRAM DIRECTOR FOR RELATIVE WORKSHOPS, MANUFACTURE OF THE VECTOR I PARACHUTE, WHICH WAS THE EQUIPMENT INVOLVED IN THE ACCIDENT, WE EXAMINED THE FOLLOWING ITEMS. A VECTOR I (MAIN), S/N 14103 AND A RAVEN (RESERVE) S/N 274918. THE FOLLOWING ARE A LIST OF THE FINDINGS: 1. THERE WAS NO DAMAGE TO THE EQUIPMENT OTHER THAN THE HARNESS WHICH HAD BEEN CUT TO EXTRACT THE BODY. 2. NEITHER THE MAIN OR RESERVE HAD BEEN DEPLOYED. 3. THE RESERVE HAD BEEN REPACKED ON FEB. 12, 2002. 4. A FUNCTIONAL CHECK OF THE MAIN CHUTE AND RESERVE CHUTE FOUND NO ABNORMALITIES.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
