## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) PILOT ATTEMPTED TO TAKE OFF FROM A GRASS AIRSTRIP AT LAKEWOOD COUNTRY CLUB AIRSTRIP (6M5), AFTER REACHING APPROXIMATELY 10 FEET ALTITUDE, THE PILOT STATED HE HAD NO RESPONSE TO THE FLIGHT CONTROLS. THE RIGHT WING DIPPED AND CONTACTED THE GROUND CAUSING THE AIRPLANE TO SPIN INTO THE GROUND. THE AIRPLANE COME TO REST APPROXIMATELY 50 FEET TO THE RIGHT AND ABOUT HALF WAY DOWN THE AIRSTRIP, IT WAS DETERMINED THAT THE AIRPLANE DID NOT HAVE SUFFICIENT AIRSPEED TO MAINTAIN FLIGHT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CHI02LA158](https://openairdocs.org/ntsb/CHI02LA158), Jun 10, 2002: Beech B35 (N352GF), Lakewood, WI, substantial damage. Probable cause: The pilot's failure to maintain control of the aircraft during takeoff when he encountered a stall after failing to maintain airspeed.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
