## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(.19) ON OCTOBER 8, 2002, ABOUT 1015 EASTERN DAYLIGHT TIME, A PIPER PA-30, N7014Y, REGISTERED TO LCM ENGINEERING INC., AND OPERATED BY AN INDIVIDUAL, IMPACTED WITH SHRUBS AT THE IMMOKALEE AIRPORT, IMMOKALEE, FLORIDA. VISUAL METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME AND NO FLIGHT PLAN WAS FILED FOR THE 14 CFR PART 91 INSTRUCTIONAL FLIGHT. THE AIRPLANE WAS SUBSTANTIALLY DAMAGED. THE COMMERCIAL-RATED PILOT-IN-COMMAND/CERTIFIED FLIGHT INSTRUCTOR, AND PRIVATE-RATED PILOT WERE NOT INJURED. THE FLIGHT WAS ORIGINATING AT THE TIME. ACCORDING TO THE PILOT-IN-COMMAND (PIC), JUST AFTER TAKEOFF AND AT AN ALTITUDE OF ABOUT 20 TO 30 FEET ABOVE THE GROUND, THE AIRPLANE'S LEFT ENGINE LOST POWER. THE AIRPLANE LANDED BACK ON THE RUNWAY, BOUNCED, VEERED LEFT, AND DEPARTED THE RUNWAY IMPACTING WITH SHRUBS. THE FAA DETERMINED THAT THE PILOT IN THE RIGHT SEAT WAS THE PIC, AND WAS GIVING INSTRUCTIONS TO THE LEFT SEAT PILOT, WHO WAS ALSO THE OWNER OF THE AIRPLANE, AND NOT MULTI-ENGINE RATED. BOTH PILOTS STATED THAT ALTHOUGH THIS WAS A TRAINING FLIGHT, SIMULATED ENGINE OUT PROCEDURES WERE NOT IN PROGRESS AT THE TIME OF THE ENGINE POWER LOSS. (-23) THE FOLLOWING FACTS WERE OBTAINED WHILE INTERVIEWING MR. LEWIS CAMERON MIDLAM AND MR. FRANCIS RICHARD BURRELLI AT IMM ON WEDNESDAY, OCTOBER 09, 2002, AT APPROXIMATELY 9:30 AM: MR. MIDLAM STATED THAT HE WAS THE PILOT IN COMMAND OF A PIPER PA-30-30 THAT WAS PERFORMING TOUCH AND GO PROCEDURES ON RUNWAY 36 AT IMM ON TUESDAY OCTOBER 8, 2002, WITH MR. FRANK BURRELLI, CFI. AT APPROXIMATELY 12:53 PM SUBJECT AIRCRAFT LOST CONTROL AND CRASHED INTO AN AREA APPROXIMATELY 350 FEET NORTHWEST OF RUNWAY 36. ACCORDING TO MR. MIDLAM, AIRCRAFT LIFTEDT TO AN ALTITUDE OF TWENTY (20'), AT AN AIRSPEED OF 105 MPH WHEN THE LEFT ENGINE FEATHERED CAUSING THE AIRCRAFT TO "VIOLENTLY" VEER TO THE LEFT. MR. MIDLAM APPLIED "HARD" RIGHT RUDDER AND WITH THE ASSISTANCE OF MR. BURRELLI PULLED BACK ON THE YOKE. THE AIRCRAFT CRASHED AND SUSTAINED SUBSTANTIAL DAMAGE TO LEFT AND RIGHT WINGS, RIGHT AILERON AND RIGHT FLAP AND RADOME. THE AIRCRAFT IS CURRENTLY FACING SOUTHWEST.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong.
