## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ACCORDING TO A WITNESS, THE ULTRALIGHT VEHICLE REMAINED OVERNIGHT ON THE RAMP AND WAS COVERED WITH HEAVY FROST. PRIOR TO DEPARTURE, THE WITNESS AND THE OPERATOR USED A BROOM TO REMOVE THE FROST FROM THE UPPER AND UNDERSIDE OF THE WING. THE OUTSIDE TEMPERATURE WAS 23 DEGREES FAHRENHEIT AND OTHER AIRCRAFT IN THE AREA HAD HEAVY FROST ADHERING TO THEM. INSPECTION OF THE WRECKAGE REVEALED THAT THE ULTRALIGHT STILL HAD HEAVY FROST ADHERED TO ITS WINGS. THE SOLE OCCUPANT SUFFERED FATAL INJURIES WHEN HE CRASHED ON TAKEOFF AT THE END OF THE RUNWAY INTO A STAND OF BIRCH TREES. A WITNESS STATED THAT THE ENGINE WAS RUNNING AT IMPACT. AN INITIAL ON SCENE INVESTIGATION INDICATED THAT THE ENGINE WAS OPERATING AT IMPACT. AN INTERVIEW WITH A UNITED STATES ULTRALIGHT ASSOCIATION BASIC FLIGHT INSTRUCTOR, AND OPERATOR OF A LOCAL ULTRALIGHT TRAINING CENTER, STATED THAT ULTRALIGHT VEHICLES WILL NOT FLY WITH ANY FROST ADHERED TO THE WINGS.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
