## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE STUDENT PILOT WAS CONDUCTING HIS FIRST SOLO FLIGHT IN A R-22 HELICOPTER AFTER BEING ENDORSED BY HIS INSTRUCTOR. THE STUDENT PILOT COMPLETED HIS SYSTEMS CHECK AND WAS WAITING FOR TWO OTHER HELICOPTERS TO LAND TO HIS LEFT FRONT. HIS AIRCRAFT WAS AT OPERATIONAL ROTOR SPEED WITH THE COLLECTIVE IN THE FULL DOWN POSITION. THE STUDENT PILOT STATES THAT FOR UNKNOWN REASONS THE HELICOPTER INITIALLY TURNED TO THE LEFT AND ROTATED RIGHT FOR TWO REVOLUTIONS WHILE BECOMING AIRBORNE ONE TO TWO FEET. THE AIRCRAFT CAME OFF THE GROUND IN A NOSE HIGH PITCH STRIKING AND BREAKING THE AFT PORTION OF THE SKID TUBES. THE AIRCRAFT MAIN ROTOR BLADES STRUCK THE GROUND PRIOR TO THE AIRCRAFT COMING TO REST ON THE RIGHT SIDE. THE PILOT EXITED THE AIRCRAFT AFTER SECURING THE ENGINE, FUEL AND ELECTRICAL SWITCHES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB MIA03LA057](https://openairdocs.org/ntsb/MIA03LA057), Feb 12, 2003: Robinson R-22 (N74842), Pompano Beach, FL, substantial damage. Probable cause: The student pilot's failure to maintain control during takeoff which resulted in in uncontrolled descent and the helicopter impacting the ground.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
