## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) SKW 3622 TAXIED OUT FOR TAKEOFF AT APPROXIMATELY 22:00 MST. THE AIRCRAFT WAS DE-ICED PRIOR TO TAKEOFF WITH TYPE 1 FLUID. TYPE 1 FLUID WAS THE ONLY TYPE AVAILABLE AT CDC. THE AIRCRAFT TAXIED BACK TO THE RAMP AND WAS DE-ICED AGAIN AT APPROXIMATELY 22:20 MST. THE 05:16Z WEATHER WAS: CALM WIND, VISIBILITY 3/4 MILE IN LIGHT SNOW AND MIST. SKW 3622 TAXIED OUT TO RUNWAY 20 AFTER THE SECOND DE-ICE AND MADE A ROLLING TAKEOFF TO AVOID GOING PAST THE DE-ICE HOLD OVER TIME OF 15 MINUTES. THERE WAS 1/2 TO 11/2 INCHES OF SNOW ON THE RUNWAY AND THE CENTER LINE WAS OBSCURED. THE PILOT ENTERED THE RUNWAY FROM THE LEFT AND FAILED TO BRING THE AIRCRAFT INTO POSITION ON THE CENTER OF THE RUNWAY. THE AIRCRAFT WAS LEFT OF CENTERLINE AND REMAINED LEFT THROUGHOUT THE TAKEOFF ROLL. JUST AFTER TAKEOFF ROLL COMMENCED THE FLIGHT CREW RECEIVED AN "ICE CONDITION" WARNING ON THE MULTI ALARM PANEL, WHICH TRIGGERS A SINGLE CHIME AND A MASTER CAUTION. THE PILOT AND FIRST OFFICER BECAME DISTRACTED BY THE "ICE CONDITION" LIGHT, AND THEIR ATTENTION WAS DIVERTED TO INSIDE THE COCKPIT. WHEN THE CREW LOOKED UP THEY DISCOVERED THAT THE AIRCRAFT WAS CLOSE TO OR SLIGHTLY OFF THE LEFT OF THE RUNWAY. THEIR ATTEMPT TO BRING THE AIRCRAFT TO THE RIGHT FAILED AND THE AIRCRAFT LEFT THE RUNWAY APPROXIMATELY 1500 FEET FROM THE TAKEOFF START POSITION.
