## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) INSTRUCTOR WITH STUDENT PILOT ON HIS SECOND INSTRUCTION FLIGHT. THE INSTRUCTOR ^PRIVACY DATA OMITTED^ WAS TEACHING THE STUDENT HOVER PRACTICE, THE AIRCRAFT WAS HOVERING AT APPROXIMATELY 8 FT. ABOVE THE GROUND. THE STUDENT WAS USING THE COLLECTIVE AND THE FOOT PEDALS, THE INSTRUCTOR WAS USING THE CYCLIC. BOTH THE INSTRUCTOR AND THE STUDENT STATED THAT THE AIRCRAFT JUST ROLLED TO THE RIGHT AND CAME STRAIGHT DOWN ON THE RIGHT SKID IMPACTING THE GROUND HARD, CAUSING THE AIRCRAFT TO ROLL TO THE RIGHT AND THE ROTOR BLADES IMPACTING THE GROUND. THE AIRCRAFT CAME TO REST ON ITS RIGHT SIDE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX03LA189](https://openairdocs.org/ntsb/LAX03LA189), Jun 9, 2003: Robinson R22 Beta II (N727DD), Phoenix, AZ, substantial damage. Probable cause: The instructor's inadequate supervision and his failure to maintain aircraft control. The wind gusts were a factor.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
