## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) DURING AN INSTRUCTIONAL FLIGHT, THE HELICOPTER LANDED HARD DURING A POWER RECOVERY FROM A PRACTICE AUTOROTATION. AS THE STUDENT WAS RECOVERING THE CFI FELT THAT SUFFICIENT POWER WASN'T BEING USED. THE CFI INSTRUCTED THE STUDENT TO ADD POWER AND TRIED TO ASSIST, BUT THE STUDENT ROLLED POWER OFF AND HELD IT THERE, APPARENTLY FREEZING ON THE THROTTLE. POST ACCIDENT INSPECTION OF THE THROTTLE CONTROLS SHOWED CONTINUITY AND FUNCTION TO BE NORMAL AND OPERATIONAL. WINDS WERE GUSTING TO 15 KNOTS. THE AIRCRAFT HIT ON THE RIGHT SKID, BOUNCED AND ROLLED OVER LEFT, LANDING ON ITS SIDE IN A BED OF CHOLLA CACTUS. BOTH OCCUPANTS RECEIVED MINOR INJURIES FROM THE CACTUS, BUT NOTHING FROM THE ACCIDENT. THE TAIL BOOM AND ROTOR WERE CUT OFF BY THE MAIN ROTOR AT IMPACT WITH THE GROUND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX03LA191](https://openairdocs.org/ntsb/LAX03LA191), Jun 11, 2003: Robinson R-22 Beta (N7508D), Phoenix, AZ, substantial damage. Probable cause: The student pilot's improper use of the throttle control, the certified flight instructor (CFI) inadequate supervision, and the student's control interference, which resulted in a loss of main rotor…

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
