## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) ON JUNE 12, 2003 APPROXIMATELY 09:00 CDT, A ROBINSON R22-BETA HELICOPTER, WAS DESTROYED DURING A HARD LANDING WHILE CONDUCTING SIMULATED AUTOROTATIONS (EMERGENCY LANDINGS) NEAR KATY, TEXAS. THE HELICOPTER WAS REGISTERED TO AND OPERATED BY HELICOPTER SERVICES, INC. OF SPRING, TEXAS. MR. EIVIND SIVERTSON A COMMERCIAL PILOT AND FLIGHT INSTRUCTOR, AND MS. YUMI SEKIGUCHI A PRIVATE PILOT AND COMMERCIAL STUDENT PILOT SUSTAINED MINOR INJURIES. VISUAL METEOROLOGICAL CONDITIONS PREVAILED, AND A FLIGHT PLAN WAS NOT FILED FOR THE 14 CODE OF FEDERAL REGULATION PART 91 FLIGHT. THE LOCAL FLIGHT DEPARTED DAVID WAYNE HOOKS AIRPORT (DWH) FOR A PIPELINE PATROL, APPROXIMATELY 0800. ENROUTE TO THE STARTING POINT OF THE PIPELINE PATROL THE FLIGHT INSTRUCTOR ELECTED TO GIVE THE PRIVATE PILOT SEVERAL SIMULATED AUTOROTATIONS. THREE AUTOROTATIONS WERE DEMONSTRATED TWO OF WHICH WERE WITHOUT INCIDENT AND THE THIRD RESULING IN A HARD LANDING DUE TO DEGRADATION OF ROTOR RPM.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB FTW03LA172](https://openairdocs.org/ntsb/FTW03LA172), Jun 12, 2003: Robinson R22 Beta (N7198H), Houston, TX, substantial damage. Probable cause: The pilot's failure to maintain rotor RPM and the improper landing flare resulting in a hard landing. A contributing factor was the prevailing high winds.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
