## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) WHILE PRACTICING AUTOROTATIONS 9 MILES SE OF CHD, THE CFI ANNOUNCED A "THROTTLE CHOP" AND THE STUDENT ENTERED AUTOROTATION @ APPROX. 600-700 FT (AGL), THEN TURNED THE HELICOPTER INTO THE WIND. THE STUDENT APPLIED AN EXCESSIVE AMOUNT OF AFT CYCLIC CAUSING THE AIRSPEED TO DECREASE RAPIDLY. AT APPROX. 350-400 FT (AGL) THE STUDENT APPLIED FORWARD CYCLIC TO REGAIN AIRSPEED, HOWEVER THIS CAUSED THE NR TO DECAY. THE CFI ANNOUNCED THAT HE HAD CONTROL OF THE HELICOPTER SEVERAL TIMES BEFORE THE STUDENT WAS FINALLY "OFF" THE CONTROLS. AT THIS POINT (30 FT AGL) THE CFI HAD BEEN UNABLE TO REGAIN THE LOST NR. THE HELICOPTER CONTACTED THE GROUND ON THE FORWARD PORTION OF THE LANDING SKIDS AT APPROX. 20-25 KIAS GROUND SPEED CAUSING THE VEHICLE TO BOUNCE UPWARD AND MAIN ROTOR BLADES STRIKING THE TAIL BOOM CAUSING IT'S SEPARATION. THE FORWARD PORTION OF THE R/H LANDING SKID THEN DUG INTO THE GROUND CAUSING THE HELICOPTER TO ROLL ONTO IT'S R/H SIDE COMING TO REST. THERE WAS NO POST-CRASH FIRE AND NO INJURY TO EITHER PILOT/OCCUPANT. THE HELICOPTER WAS DESTROYED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX03CA220](https://openairdocs.org/ntsb/LAX03CA220), Jul 1, 2003: Robinson R22 Beta (N7508E), Chandler, AZ, substantial damage. Probable cause: The student's failure to maintain adequate main rotor rpm and his failure to relinquish the controls.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
