## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT MADE THE FOLLOWING STATEMENT: THE ACCIDENT OCCURRED DURING AN INSTRUCTIONAL FLIGHT ON PRIVATE PROPERTY. AFTER INITIAL STARTUP, ALL OPERATIONAL GAUGES WERE CHECKED, ALL GAUGES WERE IN THE GREEN. THE GOVERNOR WAS ENGAGED AND THE STUDENT BROUGHT THE HELICOPTER TO A 3 FT. HOVER. THE HELICOPTER HOVERED LEFTON MY INSTRUCTIONS, WHERE THERE WAS AN INCLINE (SLOPE). THE HELICOPTER MADE A 180 DEGREE TURN AND MUST HAVE MOVED BACKWARD CATCHING THE LEFT SKID AND A SUDDEN UNCONTROLLED INPUT ON THE CYCLIC CAUSED THE HELICOPTER TO BOUNCE OFF THE UPSLOPE SKID. THIS INPUT CAUSED THE HELICOPTER TO ROLL ABOUT THE DOWNSLOPE SKID EXCEEDING THE AREA OF CRITICAL ROLLOVER, WITH THE APPLICATION OF CYCLIC CONTROL. THE DYNAMIC ROLLOVER OCCURRED IN LESS THAN TWO SECONDS. ON IMPACT, ALL SWITCHES, MIXTURE AND IGNITION WERE TURNED OFF. AN FAA INSPECTOR FROM THE (CMH) FSDO INSPECTED THE AIRCRAFT AT THE ACCIDENT SITE. CONTINUITY OF THE FLIGHT AND ENGINE CONTROLS WERE VERIFIED. FUEL WAS PRESENT IN THE FUEL TANK.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB NYC03LA147](https://openairdocs.org/ntsb/NYC03LA147), Jul 2, 2003: Robinson R-22 (N7001Z), West Liberty, OH, substantial damage. Probable cause: The student pilots failure to maintain directional control while hovering, and the CFI's delayed remedial action.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
