## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) WHILE ON A FLIGHT ORIGINATING FROM VENICE MUNICIPAL AIRPORT THE PILOT HEARD A LOUD GRINDING NOISE IN FLIGHT. A PRECAUTIONARY LANDING WAS MADE AT SORENTO SHORES AND WHILE OPERATING THE HELICOPTER AT 100% ON THE GROUND, THE COOLING FAN ASSEMBLY SHAFT BROKE. THIS ALLOWED THE ROTATING FAN ASSY TO IMPACT THE SHROUD ASSEMBLY CAUSING DAMAGE. INVESTIGATION DID NOT INDICATE ANY IMPROPER MAINTENANCE OR OPERATION. A MALFUNCTION DEFECT REPORT (FAA FORM 8010-4) WAS FILED ON THE COOLING FAN SHAFT P/N A642-2.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
