## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) THE PILOT FORGOT OR OVERLOOKED REMOVING THE RIGHT AILERON TO FLAP GUST LOCK DURING THE PREFLIGHT. AT ROTATION SPEED THE PILOT PUT SOME FLAPS DOWN TO AID THE TAKEOFF. THIS CAUSED THE RIGHT AILERON TO COME DOWN WITH THE FLAPS CAUSING A TURN TO THE LEFT WHICH CAUSED THE LEFT WING TIP TO STRIKE THE GROUND SPINNING THE AIRCRAFT AROUND ON THE GROUND. THE AIRCRAFT WAS DESTROYED. THERE WAS NO FIRE. THIS REPORT CLOSED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB LAX03LA230](https://openairdocs.org/ntsb/LAX03LA230), Jul 10, 2003: Aero Commander S2R (N8912Q), Tulelake, CA, substantial damage. Probable cause: The pilot's inadequate preflight inspection and failure to remove the gust lock, which resulted in his loss of airplane control.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
