## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

(-23) PHILIP KEOHANE DEPARTED FROM OWD IN N5944U AND WAS DESCRIBED BY OWD TWR CHIEF AS "FOGGY AND DISORIENTED", ASKED TO DEPART WEST AND FLEW 5 NM EAST THEN TURNED AND CLIMBED INTO THE CLASS B WITHOUT A CLEARANCE. HE ARRIVED AT SFZ AND MADE WHAT WAS DESCRIBED BY A DPE AS A DANGEROUS APPROACH NARROWLY MISSING ANOTHER AIRCRAFT AND THE VASI LIGHTS ON THE RIGHT OF THE RUNWAY AND THEN EXECUTED A GO AROUND. HIS SECOND APPROACH WAS DESCRIBED BY TWO WITNESS STATEMENTS AS OUT OF CONTROL. HE ATTEMPTED TO LAND 500-600' BEHIND ANOTHER AIRCRAFT ON THE RUNWAY AND BOUNCED A NUMBER OF TIMES BEFORE THE AIRCRAFT STAYED ON THE GROUND AND VEERED OFF THE LEFT SIDE OF THE RUNWAY, STRIKING A RUNWAY LIGHT AND DEMOLISHING IT. PILOT THEN PROCEEDED TO TAXI DOWN THE GRASS WITH A FLAT NOSEWHEEL AND MADE SEVERAL ATTEMPTS TO GET BACK ONTO THE RUNWAY. HE PROCEEDED TO THE END OF THE RUNWAY, TWO RIGHT TURNS ONTO THE PARALLEL TAXIWAY AND TAXIED TO THE RAMP AREA WHERE AIRPORT PERSONNEL MET HIM AND HE SHUTDOWN THE AIRCRAFT. DAMAGE INCLUDED A FLAT NOSEWHEEL, BROKEN LEFT WHEEL PANT, AND DAMAGED LEFT WINGTIP. ROGER TAYLOR, LINE SERVICE SUPERVISOR, HELPED THE PILOT OUT OF THE AIRCRAFT AND SAID THAT HE HAD A NOTICEABLE LIMP.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
